Work shall be charged based on time (port to port) and material as follows:

• Technician Rate: $75.00/per man-hr. (2-man crew), cost of material(s) plus 25.0%; Minimum Rate is $250.00
• Emergency Repair Rate: $112.50/per man-hr. (2-man crew), cost of material(s) plus 25.0%; Minimum Rate is $500.00
• After-Hour / Weekend Rate: $112.50/per man-hr. (2-man crew), cost of material(s) plus 25.0%; Minimum Rate is $500.00
Terms of this Agreement:
I am the Owner and/or an Authorized Representative (herein after “Owner”) of this Property Address at which the specified work is to be performed. I hereby authorize A&H Contracting Group, LLC. (“A&H”) to perform said work, to use such labor, materials, and equipment as deemed necessary by “A&H”. Should a credit card be used to pay for such service(s), a 3.0% charge will be added.
The repairs performed on-site by “A&H” are based upon visual investigations at the specified area of which the problem was reported. Prior to commencing with work, unless otherwise requested and paid for by the customer in advance, testing to determine the internal content will not be performed. “A&H” cannot guarantee that no additional problems will be encountered during the repair process.
I hereby authorize, A&H Contracting Group, LLC., (“A&H”) to perform such said work in accordance with the terms and conditions as set forth above, attest that I am empowered to authorize such service on the property premises listed herein, and I agree to pay per the Terms. If such repair is more significant than predictable, “A&H” will provide a written Estimate is required in place of this Repair Work Authorization.
I understand and acknowledge that by accepting this work I am providing my electronic signature for A&H Contracting Group, LLC. (“A&H”) to proceed with this work in accordance with any, and all, applicable laws specifically including, but not limited to, the Federal Electronic Signatures in Global and National Commerce Act and/or Chapter 668, Florida Statutes.
